"Entry Level Exit Wounds" as written by and Mike Okubo Brandon Butler....
Dirt habits and dirty lives
She can't hold back
She must fulfill the end of the addiction

Hunting, sweating, freezing
Just another day in the life of a dead girl

She can't live without her shit
Rock bottom is just around the corner
And she has a first, a first class ticket
Test the vein, prep it
Tie the knot, make the blood clot
Needle in. "Oh, what sin!", they all say

Just wait there's more.
More from this whore?
Oh yes, there is
She ruined ruined her ruined her kids

In a world where few, few can understand
She's full of full of fucking pity
In this life who is she, who is she kidding
Now she has entry level wounds, entry level exit wounds
Now she has entry level exit wounds
Her face is missing

General CommentThe song is obviously about heroin addiction.
"Hunting, sweating, freezing" = Withdrawls from heroin/drugs.
    "Test the vein, prep it...
Tie the knot, make the blood clot... = Prepping a vein to inject one's self with heroin.
    Needle in."
"Now she has entry level exit wounds" = Entry Level Exit Wounds are the marks left from a needle becuase the needle pulls the skin back when it comes out making an exit wound.

General CommentPretty much certain this song is about drug addiction and either in the context of the song writers girlfriend or mother and how the addiction has ruined the lives of her kids(the writer being one of the kids).

The entire album expresses issues that can cause the "common man's collapse" and drug addiction certainty fits into it. The idea behind the title of the song meaning(to me at least) maybe means these wounds from his mothers actions are only the beginning of the death of him.
