This song is clearly about being addicted to heroin and what being addicted to it takes away from you.
Shots down and riding the bomb is a metaphor for how dangerous it is to be a heroin addict
The band's struggles with drug dependency (especially during this time) is heavily documented and confirmed by the band members of the time. As @hre2stay commented there are references throughout this song to drug use and it appears to clearly be dealing with the addictive nature of drugs, their effects on an individual (their erratic behaviour) and the challenges with breaking such a cycle (if indeed they want to break that cycle).
