I think your first comment is closer to being accurate. The singer/song writers state "Millions of eyes can see, yet why am i so blind!? When the someone else is me, its unkind its unkind". I believe hes referring to the girl toying with him and using him. He wants something deeper with her, thats why he allows himself to be as a puppet (even though for her fun and games) as long as it makes her happy. But he knows deep down that she doesnt really want to be serious with him and thats what makes him.

I think this song is about 2 people who had a relationship, perhaps in college. It obviously ended for some reason - maybe it just faded out or ended horribly. For whatever reason they probably haven't really talked since then and meet somewhere (probably a party) a few years later. Its funny that they were introduced because they clearly had a past the simple "I think we've met before" is like them keeping their relationship part of the past and not a response like "oh my gosh I knew you in college!!" Etc. The driving in silence is like not really knowing what to say, not necessarily awkward but just remembering things from the past. The scar being a fleck is kind of like the relationship is something she isn't going to forget and maybe is frequently reminded about despite never seeing this guy. She's moved on and found real love and its ok.
