This is a song about a doctor that's torturing/murdering people. Teehee
I believe the ending is sung from the position of the dead corpse lol. The flies and rats eating the body perhaps?

Hmm now that I think of it I remember reading a connection between this song and an actual incident that happened. I could be wrong on that however. Sometimes I dream things up.
