it means something different to everyone, but to me it resonates bacause of my struggles with drug addiction. I used to work in a hospital and saw addicts come in nightly strung out and felt disgust. You never think it'll happen to you.

For me it's about personal growth. "You" could be a personification of some negative quality such as a bad habit to be overcome, or "the opponent within." This personification seems to be used in "Bruises" as well.
