For me, this song is an affirmation of wedding vows, through better and worse - even when we "come undone". That's what I feel from the song. And I read that Simon wrote this as a gift to his wife, so that seems about right to me.
Another person gave a quite different interpretation based on the official video for the song; however, videos are created with quite a bit of artistic license to help sell songs. Just look at the video for "The Chauffeur".
Songs evolve from inspiration to commercial viability; I sort of think the "falling apart at the seams" and "taking my heart to pieces" are commercial embellishments that sound good, but maybe stray slightly from what I think the original intent of the song was about - an affirmation of marriage despite whatever turmoil life has presented.
It seems to me that Duran Duran's very best songs (lyrics are the most important aspect of a song to me) are those that have a personal connection to Simon. This song, along with "Ordinary World" and "The Chauffeur" all do, from what I've read, anyway.
