I don't think this song is about self harm at all! Here's my interpretation of the song:

Push me up against the wall
Young kentucky girl in a push-up bra
Fallin' all over myself
To lick your heart and taste your health
--Anthony doesnt feel as young as he used to be; he tries to get rid of this with this young, healthy girl to make himself feel better

Blood loss in a bathroom stall
Sounthern girl with a scarlet drawl
Wave goodbye to ma and pa
--a young girl who has just had her first period, she's growing up and now the time is moving closer when she has to leave her parents and make it on her own.

Soft spoken with a broken jaw
Step outside but not to brawl
Autumn's sweet we call it fall
I'll make it to the moon if i have to crawl
--often when people get older, they're not as quick to get loud and violent-they tend to think before they act, that describes the first three lines, but I'm not sure what the fourth one means

Scar tissue the I wish you saw
Sarcastic Mister know it all
close your eyes and I'll kiss you cause'
With the bird I'll share
This loney view
--he wishes that people would see that he isnt really as tough as he appears to be, perhaps 'sarcastic mister know it all' is a description of himself, or at least the image of himself that everyone seems to have
he just wants to have someone to love (close your eyes and I'll kiss you)
and lastly, he feels lonely--he feels that no one on earth can understand him-the birds that fly high in the sky, who see everything from a totally different perspective are the only ones who really know his pain.

Overall, I think this song is about how anthony is getting older, he's sad because he still hasn't found anyone who he really connects with and who really understands him. He's also worried, because he thinks that he wont find that person in time, possibly because of his false image of toughness of him that he's put out, he wants people to know that he's only human and wants to be loved just like everyone else.

--was that a long enough description for ya?? It took a loooooong time thinking about this song for me to think that up--it could be totally wrong of course, but that's just what I think this song is about, so lemme know what you people think about it

ok I didnt want to 20 years reading all these comments, but when I read the line 'It's OK to eat fish, 'cause they don't have any feelings' it came to my mind that Kurt's zodiac is pisces...I don't know what that might have to do with the meaning of the song, but it was just weird that I thought of that

In an interview I read, Kurt said that it was about "Classic alienation, I guess. Every time I go through songs I have to change my story, because I'm as lost as anyone else. For the most part, I write songs from pieces of poetry thrown together. When I write poetry it's not usually thematic at all. I have plenty of notebooks, and when it comes time to write lyrics, I just steal from my poems." I just heard this song for the first time a little while ago and after I burned it on a cd I havent stopped playing it....I can just totally relate-it's weird how sometimes you can totally connect to a song like that.

Have you written about this stuff at all? The lyrics on "Serve the Servants" sound autobiographical.

Kurt Cobain: Yeah. It's the first time I've ever really dealt with parental issues. I've hardly ever written anything that obviously personal.
