I think Katy is the heroine. The narrator is a down and out of luck dude who is broke and bummed out when his old friend shows up: Dr. Wu. I am not sure whether Dr. Wu is supposed to be a real person or a metaphor for something else. Lovely song and one of my favorites.

I always thought this song is about being (more like perceiving yourself) unattractive. I'm a relatively short man, and it was a disqualifying feature for some of the girls I was interested in. Hence my body is holding me from being with someone I love. And the lines "my mind holds the key" represented the personality. So if I'm able to talk the person who I see as out of my league, maybe I could charm them so that they can appreciate me despite some appearance issues that I have (or think I have).
