I think this song is about having a friendship with a self destructive person who doesn't have self awareness and follows a detrimental path for them and the relationships with other people.. Possibly it's a metaphor for narcissistic personality traits, which are notoriously difficult to work with.
The outro ("boy...") hints at aiding this person, medicating them and giving them opportunities, but they still follow the unenlightened path of greed or self indulgence. One of Brand New's best songs IMO
