I think this song is about a relationship that started when the singer and his significant other were very young, perhaps in high school or even college. They had what they thought was an amazing relationship ("So young and caught up in it.")

However, as they got older, they both changed and began to drift apart, through no fault of their own ("The way you are / It's not your fault; you knew that"). The singer feels that the other person is completely different than the girl he fell in love with. He laments that neither of them could see it coming ("Too far to see how things can be") and that "I thought I knew who you were". The best thing for both of them to do is to end the relationship, since they are both completely different people now ("Just trust that you'll be strong, and just let your life move on."). Still, the singer misses what he had "deep inside."
