I love this song. I think that it's about the lead singer's past drug addictions (scars on my skin) and addiction in general. Someone in the shadows and gullies of life crying out for help, remembering somewhere that the savior of the world (Jesus) still remembers them and is still right there by their side to lead them back into the light when they're at their lowest point.
