To me this song is talking about drugs, as do many of Alice in Chains songs. Layne Staley, as many know, was an addict who battled drugs for most of his life and sadly lost the war. This song is talking about trying to come out of the dark hole and get back on track, but he keeps making the same mistake, like the "same old trip it was back then". This doesn't only have to relate to drug addiction, it could relate to everyday life in a bad rut and trying to get out, but you seem to fall right back in.

No, this song was written by Cantrell as a hommage to Andrew Wood, late singer of Mothe Love Bone (hence the title 'would'). As far as the drugs relation, you're pretty much right, as Wood died of an overdose (see Whole Night Thing by Temple of the Dog).
