I really love this song, I think it captures the frustration of being in a relationship (or having been in a relationship) with a partner whose actions and behaviour are destructive to it, and who never learns from the mistakes they've made. The person's trying desperately to salvage what they have left in the relationship and asking the partner to do the same (hence the 'hold on, hold on, hold on) because they don't want things to fall apart worse than they already have.

Just a lyric edit: In the second verse, the first 'chains' is meant to be 'trains' (could have sworn I'd written it right when I typed it up originally)!
