this has got to be the best suicidal tendencies song ever. period. it's funny, it's sadistic (sort of), and, musically, it's some hard shit. I especially like how the music builds up at the end of each of the verses, as if mike's anger is just continually building; then when it hits the chorus, it just lets loose...then calms back down until the end of that verse.
the part about him getting hit by a car isn't about suicide, as c_o_c_o pointed out. I think it's just a typical "woe-is-me" statement, after giving up on trying to convince the people that he's not crazy.
