i think its talkin about the many unworthy leaders of America ('for those who can afford it'-- usually the candidates with the most money and popularity win) and negative media (like tv) and how its destroying and desperately bringing America as a society to a lower level. 'One nation above God himself'-- how America's been slowly shutting God out of society and life (as if we dont need him or that we're above him)
'American sheets on the deathbed' -- if you live by what's become the widely accepted American lifestyle (without God), you're gonna die spiritually
