    My InterpretationSounds like the unspeakable scene is a a suicide. Inviting the suicidee to 'strike a nerve and leave a scar [he] won't forget' is challenging him to knife his wrist, feel the pain and realise he doesn't want to go through with it. The last verse is a bit ambiguous though - 'just when you think that you're okay, you start hearing voices' could mean either that the attempt was interrupted or that he went through with it.
