There she goes out the back door my birdlike eye scans the welfare cart for a new source of inspiration, a point of interest until I reach my final destination.

    Addicted to heroin, even with physical body harm she can't stop, giving the reader a good backstory but an even better one to someone who is addicted. He's asking himselfhow long she well last as he had seen the end and back.

    Lost weight because she choose to do heroin over food, she lost weight and currently going through withdrawals.

    He's writing about someone else that shares the same addiction as him and all he did was point out the bad points of her (the user).

    This is slang for someone who only had one vain left to inject. "He broke his good leg in the plunge", he busted his last useable vein for injection, hence he most likely has no money left and now will be heroin sick.

    "Yeah, he’d make a good character study or his he busy studying about me?"

    The writer is losing his mind, becoming paranoid, creating false sense of people's motives.

    After writing these lyrics, the alter writer (possibly in the main writers mind) thoughts he started using again or the alter writer just assumed to write about the singer due to his previous track record with heroin.
