I think your first comment is closer to being accurate. The singer/song writers state "Millions of eyes can see, yet why am i so blind!? When the someone else is me, its unkind its unkind". I believe hes referring to the girl toying with him and using him. He wants something deeper with her, thats why he allows himself to be as a puppet (even though for her fun and games) as long as it makes her happy. But he knows deep down that she doesnt really want to be serious with him and thats what makes him.

This song is written as the perspective of the boys in the street, as a whole, and what path they are going to choose as they get older and grow into men. I think that she was probably mentally ill (I think bipolar disorder is probably right on) and that she committed suicide, probably by drowning because of the plethora of water references (even the name Maggie, sort of Margaret, which is derived from the word for "pearl," which is appropriate, I think from his persepective, plus mar=sea). I also think that the fact that Maggie "walks" rather than swims after death.
And being thin doesn't automatically mean that she was sick physically, it's just as likely because of being on edge mentally.
