This is a hauntingly beautiful song about introspection, specifically about looking back at a relationship that started bad and ended so poorly, that the narrator wants to go back to the very beginning and tell himself to not even travel down that road. I believe that the relationship started poorly because of the lines:

So, the first night was not a great start, but the narrator pursued the relationship and eventually both overcame the rough start to fall in love with each other:

Like many relationships that turn sour, it was not a quick decline, but a gradual one where the narrator and their partner fall out of love and gradually grow apart

I'm thinking the narrator is a partier who looks for one night stands rather than a real relationship. The "quick fix" is what she wants, but once she gets it, she realizes that there's more to a relationship than the sex. She fears that she'll never learn from her mistakes and just keep repeating them ("This will be the death of me") She may often find herself in these senarios with taken men/women, or she just hates the situation so much she feels guilty for it- making her the "Queen of Apology" (think about it- what do you do when you are guilty? You apologize) She tries to fight her way out of this trap, but keeps falling back in.
this song is so awesome. especailly the remix. its pretty obviously about a girl in an abusive relationship. she loves him and he keeps apologising saying it will never happen again wen he hits her but it does. and she continually takes him back

i think this song is about doing drugs. i think when she takes it she feels so good its got her on her knees again. but she knows that it is bad for her. and she knows she has no one to blame her self for it. but when she doest do it she has withdrawls and she wonders if she will ever get rid of that feeling. will she ever be the same again
I heard the Re-mix and I had to just analyze the song and I like all the other analysis so far but I would like to take this into happier terms
What this song is about is about love of course but it's about unrequited love at first then it being returned. First it talks about "Quick Fixes" and "Heartbeats" and what that can mean is that she did the quick fix and gave up and noticed that her heart still beated but she never truely gave up because her love could still drive her "On her knees" or under the feeling. For the next line she doesn't realise she is in love again till some time goes by and she thats when she apoligies for the first time. For the second verse the she is trying to give up again before her emotions engulf her again. For the third verse she is talking about how she really has no-one to blame but her self and how she can't call the person an Enemy. As for the Monkey it is the addiction of love for him. for the Fifth verse onword very much the same as the second one. As for the Line "This will be the death of me" she is talking about her split between loving him and trying to give up again.
I think it's only about the singer's feelings of remorse/regret over something that we don't know. The song isn't specific. But it never leaves her, and it especially hits her hard when she sees a certain someone. Who she is, again, not specific about.

Definitely about addiction. It's natural to fall into a cycle of self-blame and denial (apologies).
The fact is that addiction, for many people, never -really- goes away. It can take a long time to be able to get through a day without the shakes, and that time can be more physically painful than anything else they've experienced.
And even then, for many people not a day will go by that they won't have to fight that feeling, and they do fight it knowing that all they have to do is smoke/drink/poke one quick fix and the pain would go away.
And of course, if she doesn't quit, she wakes up knowing what she did thas shortened her life. It will kill her if she keeps going, but maybe it's worse to quit.
Addiction isn't fair.
