I think it's just describing his state at one point...I guess...
"with you on my mind"...kinda straightforward. so he's thinking about this person. Because he likes them.
"and my heart held in your hands"...the other person doesn't yet know how they feel, or won't tell him. he has laid himself on the line...he is basically giving his heart to this person and telling them to make up their mind.
"screaming break me"...waiting hurts. he wants the decision fast.

But I also feel like in my own current situation with a horrible boyfriend it's saying that...
someone has his heart and his relationship with a lover or someone isn't going good and the other person is thinking about breaking it off and he doesn't want that to happen and it's hurting him so much so he's just waiting for the other person to break him
so he can get over it.
