    General CommentI think it's showing how people only pay attention to people's problems after they've acted out severely--by hurting themselves or others, etc. They never really listen when people just talk, so, to get yourself help, sometimes it's necessary to shout.

    People ignore their problems ("sword in your side...") until it's too late and they've allowed it to grow and worsen.
