This is actually really troubling to me, as a guy who was sent away as a teenager. I was lucky enough to have parents and other adults who cared enough about me to send me to an amazing place, and it saved my life. I absolutely adore this song, but I really hope that Darnielle (and parents who are terrified for/of their child and teenagers who are in a really black place and don't know where to turn) that the system is changing. There are some really great places for kids and teens nowadays; the program that I went to was so amazing that the only way I could imagine giving back to the system that helped me was to go to work for it. I have a job at a (different) residential treatment facility, and while it's true that in the past the system was broken and had some terrible places that did things like punish kids for dreaming their dreams, now some of these places are fixing the system from the inside. There are bright places full of love, devoted completely to helping kids turn their lives around. One of these programs is the only reason that I have a life and a a future that I can do whatever I want with. They taught me to hope, and start dreaming my dreams again, after I felt like I had been spending years living inside a dark hole inside my own head.

None of them are rational actions. They aren't particularly loving, either. But each is meant, to a certain extent, to be motivated by love. This song illustrates love as a force that can be destructive, when people do things terrible for all the right reasons.

It's him, singing about how he's not going to find love in a hole (the hole being a euphemism for sex with this girl). The entire album is about a terrible relationship and its terrible end; he had his heart broken, so he's trying to mend it with plenty of sex. The lyrics are chiding himself, saying that he's not going to find love in one night stands with women he meets at clubs.
This first-person perspective is most apparent in the verse beginning "I'm drunk, and you're probably on pills."

Seeing this performed when AFP was touring in America with the Danger Ensemble was incredible. Immensely powerful and moving, and it was tied amazingly well to the next song performed, Guitar Hero.
Then, Amanda cut her head while fake-playing guitar. It doesn't get much more Rock than that.

I don't really see how this is about smoking. I mean, the smoking reference midway through the song is just saying that she'll live a good long time, if she doesn't smoke. The point of the songs seems to me to be that she's waiting and trying to build the courage to confront someone she loves with the fact that she cares about them. It's Amanda perhaps resigning herself to not having that courage, and trying to enjoy herself in spite of not being able to confront them ("can't we just wait together/you bring the smokes, I'll bring the beer"). An absolutely beautiful song.

Urchinau—No, I think they're right. The line is "if not now, I know I can wait, like Lucas and the Locusts." George Lucas waited thirty years to follow up on his immensely successful Sci-Fi franchise, and the Locusts in Gears of War waited under cities to erupt from the ground and assault the humans on Emergence Day. I mean, your reference includes a Lucas and locusts, but it doesn't involve any sort of waiting.
I mean, it could easily be a dual reference, but it makes more sense in context to think of George and Gears before the film Lucas.

Maybe the title IS a Harry Potter reference, but I always felt that it's referencing the phrase itself, not the pop-culture curse. Rowling herself has described it as meaning "let it be destroyed" or "destroy it," but I've also heard that it means just "bring me the cadaver." I think the first is more accurate, and it definitely applies to the song. Either way, I don't think (or like the idea that) it's a reference to Harry Potter.
